PLEASE NOTE:   All new patients are required to print out the following forms, policies, and acknowledgements, read each document, and fill out each document as required. Please be sure to bring all of the documents with you to the office for your first scheduled appointment. This will save you time and help us keep to our appointment schedule.

Failure to bring the completed documents with you or not having the documents completed prior to arriving for your appointment, will cause an increase in the amount of time required for your appointment because all of the documents MUST be completed before seeing the doctor.
